How to Use the Review Dashboard

Step-by-step guide for completing and submitting annual employee reviews.

Signing In
  1. Go to sh-review-dashboard.pages.dev
  2. Select your name from the dropdown
  3. Enter your 6-digit PIN and click Sign In
  4. Your session stays active for 7 days — you won't need to log in again unless it expires
Forgot your PIN? Contact Sao (Admin) to have it reset.

Completing a Review
  1. From the Home page, click the review form for the role you're evaluating (DA Annual Review or RCM Annual Review)
  2. Fill in the employee's name, office, tier/level, and review date at the top
  3. Score each section using the 0–5 buttons or dropdowns — the overall score and raise calculation update live as you go
  4. Check the tier advancement box if the employee is ready to move to the next tier
  5. Enter your name in the Reviewer Signature field at the bottom
  6. Click Submit for Approval — this creates a ClickUp task and emails leadership a summary
Important: Make sure you are signed in before starting. The form must be opened from the Home page (not bookmarked directly) to link your session for draft saving.

Saving & Resuming Drafts
  1. Click 💾 Save Draft at the top or bottom of the form at any time — your progress is saved to the cloud instantly
  2. Each form fill creates its own separate draft — resetting the form starts a fresh draft
  3. To resume later, go to Records, find the draft row, and click Resume — the form reopens exactly where you left off
  4. Drafts expire after 90 days of inactivity
Multiple reviews: You can save a draft for one employee, reset the form, and start a new draft for another. Each saves independently.

Review Records
  1. The Records tab shows all your drafts (in progress) and submitted reviews
  2. Use the search bar or filters to find a specific employee or form type
  3. Add internal notes in the Comments column — click Save to persist them
  4. Click View PDF to open the submitted review document
  5. Click ClickUp ↗ to open the task in ClickUp for approval workflow
Admin only: The Re-edit button on submitted reviews creates a new draft pre-filled with the original scores for corrections.
👥
Profiles
  1. The Profiles tab shows a history of all reviews you have submitted
  2. Each submitted review is automatically saved to your profile — no extra steps needed
  3. Admin can view all reviewers' profiles to track activity across the team
  4. OMs and Regionals only see their own profile
